Google sits awkwardly in the middle. What the Features Actually Mean
Spec sheets are written to be intimidating. Here's the translation.
Optical heart rate — a green LED reading blood flow at your wrist or finger. Fine at rest and steady cardio, less reliable during weights and intervals where the wrist moves a lot. Every device here has it.
ECG — electrodes measuring actual electrical heart activity, used to screen for atrial fibrillation. This is a genuinely different sensor, not a software feature. Apple Watch Series 11 and Ultra 3, Garmin Forerunner 970, Samsung Watch9 and Ultra2, Pixel Watch, and Whoop MG have it. Apple Watch SE 3 and the cheaper Garmins do not.
HRV (heart rate variability) — the variation in time between beats, measured overnight. Higher generally means better recovered. It's the input for basically every "recovery" score on the market, and it's only useful compared against your own baseline. Your HRV versus someone else's means nothing.

Multi-band (dual-frequency) GPS — the watch listens on two satellite frequencies, L1 and L5, instead of one, which fixes the position drift you get in cities and under tree cover. If you run on roads or a track, single-band is fine. If you run trails or between tall buildings and care that your pace isn't garbage, this is the feature to pay for. Notably: Apple Watch Series 11 does not have it — only the Ultra 3 does.
SpO2 / pulse ox — blood oxygen saturation, mostly used for altitude and sleep-breathing signals. One quirk to know: on US Apple Watches the reading is taken by the watch but calculated and displayed on your paired iPhone in the Health app. That was a workaround for a patent fight Apple has since won — the ITC closed the matter in April 2026 — but as of now the iPhone-side flow is still how it works.

Garmin's advantage isn't sensors, it's the training brain. Training readiness, daily suggested workouts, race predictors, running power, actual maps on the higher models. If your goal is a faster 5K or a first marathon, nothing else here is close.
The 2026 entry-level models changed the math. The Forerunner 70 at $249.99 and 170 at $299.99 both got AMOLED screens and the full physiology stack — training readiness, HRV, running dynamics, sleep tracking — which used to mean spending $500. Apple's pitch is that it's a good watch that also does health well, and that's fair. Know the tiers. SE 3 at $249 does not have ECG, blood oxygen, or hypertension notifications — but it did gain the always-on display and sleep score, which makes it the best cheap sleep tracker Apple has ever sold. Ultra 3 is the only Apple Watch with dual-frequency GPS and satellite messaging, and the only one built for diving and multi-day use.
Best for: iPhone users who want one device for everything, and anyone who wants ECG and hypertension screening without a subscription.
Skip it if: You're on Android — it simply won't pair — or you want a watch you charge weekly instead of nightly.
Google Pixel Watch 5 and Fitbit Air — the health-AI play

Google is pushing hardest on health interpretation. Pixel Watch 5 has ECG, dual-frequency GPS, and Gemini on the wrist today. The headline additions — blood pressure pattern tracking, insulin resistance estimates from 30 days of sensor data, sleep breathing quality, and breathing emergency detection — are announced as "rolling out soon" rather than shipping, and breathing emergency detection has not been cleared by the FDA and is launching in select EU markets first. Buy it for what it does now, not the roadmap.
The genuinely interesting product is the Fitbit Air at $99.99 — a screenless band with 24/7 heart rate, AFib alerts, SpO2, HRV, and sleep stages, about a week of battery, and it works with both iPhone and Android. It's the cheapest honest entry point in this entire post.
Best for: Android users who want the deepest health-coaching software, or anyone who wants real tracking for a hundred bucks.
Skip it if: You have an iPhone and want the watch — Pixel Watch doesn't pair with iOS at all. Fitbit Air does. And go in knowing the coaching layer is a recurring $99.99 a year.
Samsung Galaxy Watch9 and Watch Ultra2 — the free-software option

Samsung has the most sensors for the money: ECG, FDA-cleared sleep apnea detection, dual-frequency GPS, and the only body composition measurement on a mainstream watch. When Google moved its coaching behind a paywall in 2026, Samsung shipped its Health overhaul explicitly free — Vitals, Heart Health Score, Daily Cardio Load, Fitness Index, all included.
There's one real catch, and it's bigger than the spec sheet admits: the clinical features — ECG, blood pressure, irregular rhythm notifications, sleep apnea detection — run through Samsung Health Monitor, which requires a compatible Samsung Galaxy phone. Oura added Live Activity Tracking in June 2026, so it now does real-time pace, distance and heart rate — but the GPS comes from your phone, not the ring. It's a recovery and sleep instrument that tolerates workouts, not a training device.

What Matters Less Than the Marketing Says
Sleep stage precision. Every one of these devices will confidently tell you how much REM you got. The validation research says take that with salt. A 2021 study in SLEEP tested seven consumer sleep trackers — four wearables and three bedside sensors — against clinical polysomnography and found sensitivity for detecting sleep was high (all ≥0.93) but specificity for detecting wake was low, 0.18 to 0.54, and that "device sleep stage assessments were inconsistent."
